Changing culture will be high on the op risk agenda in 2015 and the years ahead – and it will not be an easy task for anyone
This time last year, we expressed the hope that the 2015 Top 10 operational risk list would contain fewer examples of tiny failures growing into major problems than the 2014 list did.
